You want to know if the college is NCAA Division I, II, or III or NAIA Division I or II.

2. What level of athletic scholarship do you offer and how might I be considered?

Very important to know the actual range of dollars possible. MOST athletic scholarships are only partial scholarships.

3. What are my chances, realistically, that I will play right away in my first year?

You want to know, honestly, if you will be able to play right away and how playing time is awarded.

4. What are the in-season and out-of-season expectations for academics and training?

College athletics is a big commitment of time. You need to know for sure what to expect going in so you can determine if you will be able to devote the time you need for studying depending on the academic caliber of the institution.

5. What is the head coach's preferred style of play?

If you are a running back and the head coach likes to pass 70% of the time, you need to know that going in!

6. What is the head coach's instruction style? How do they dole out criticism?

It's important for you to know the coach's style. At one of the end of the spectrum is if the coach is "yeller" that likes to confront and challenge and at the other end is a coach that offers tons of positive reinforcement and only criticizes in one-on-one meetings.

7. How is the relationship with college faculty and the head coach?

You want to have some idea going in if your professors have a good relationship with your coach. If they don't, they can make your life miserable, especially when it comes to missed class time due to travel associated with away games.
